Please click here if you are a HealthPartners Network Provider.The goal of HealthPartners Medication Therapy Management (MTM) program is to increase medication effectiveness for Medicare Part D and Minnesota Health Care Programs (GAMC, MA, MinnesotaCare, MSHO, and Minnesota Senior Care) recipients through improved medication use, cost-effective medication use and a decrease of harmful drug influences. What is Medication Therapy Management?
Medication therapy management (MTM) is a partnership of the pharmacist, the patient, and other health professionals that promotes the safe and effective use of medications and helps patients achieve the targeted outcomes from medication therapy. Your pharmacist will take you through a few easy steps:
2. Your pharmacist will collect your medical information and enter it into a secure computer program that organizes your medical history. You will asked to come into the pharmacy to take a look at your medical history in person.

Once the information is collected and organized, your pharmacist will go through the information with you and provide you with a report detailing where you may be able to improve your use of medication. Your pharmacist can review the report with your physician to determine which recommendations are best for you.
What is included in this program?
• You can receive a converstion to talk about your medication with a conveniently located pharmacist.
• Your pharmacist will help you:
o Understand your medications and answer any questions about your medicines or medical conditions.
o Make sure your drugs are safe to use together.
o Make sure you are not having any side effects from your medications.
o Take your medications correctly to minimize side effects.
o See if there are any less expensive medication alternatives.
o Simplify your medication regimen.
How do I sign up?
• Your assigned pharmacy will call you to set up a one-to-one meeting. (You can still pick up your medications at the pharmacy you’re currently using.)
• If you want to have a one-to-one consultation at another participating pharmacy or clinic, please call HealthPartners at 1-877-296-9670.
What to bring to the appointment?
• A signed permission form which allows HealthPartners to release your medication history to the pharmacist.
• All of your prescribed medications, including any over-the-counter drugs, vitamins and herbal supplements you commonly take.
HealthPartners offers you this service because they care about their members. However, if you do not wish to participate, you can opt out of the program or simply decline the interactive consultation. If you decline the consultation, HealthPartners will maintain its commitment to your health by sending a quarterly review of your medication to your doctor. If you opt out of the program entirely, you will not be contacted about this program at all for the remainder of the calendar year.
